Practitioners who are no longer on the register

If your name is no longer on the register of practitioners your must apply to have your name restored. You will need to confirm your identity to be given access to your myOTBNZ account. 

Once you have access to your account you will be able to make an application to restore your registration status. You will need to pay the required fee.
